Reviews and verifies orders, manages drug therapy protocols, educates staff/patients, and supports quality and regulatory standards. Reports to the Director or Manager of Department.Essential Functions:Manages drug therapy protocols as part of an interdisciplinary team.Reviews, interprets, and evaluates medication orders to identify, prevent, and resolve drug-related problems.Provides drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.Documents interventions and outcomes for drug therapy quality improvement.Verifies medication orders for accuracy/completeness prior to entry; checks filled medications prepared by technicians; maintains proficiency with pharmacy automated systems and software.Manages patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.Participates in performance improvement and required continuing education.Performs other duties as assigned.Qualifications and requirements:Education: Doctor of Pharmacy.License: Current license with the state Board of Pharmacy.Certifications: BCLS required; ACLS preferred. Skills: Critical thinking, decisive judgment, and ability to work with minimal supervision in stressful environments.
