A legacy that will make your life better as well.As a Hospice RN Field Nurse, you will plan, organize, and direct home care services with an emphasis on community health education and experience. You will build from resources of the community to plan and direct services to meet the needs of patients and families within their homes and communities.Some of your primary responsibilities will include:Completing an initial assessment of patient and family to determine home care needs. Providing a complete physical assessment and history of current and previous illness(es)Supervising and directing the Licensed Practical Nurse (LPN) and/or the Home Health Aide in implementing the patient care planDeveloping patient education plans, as needed, and directing the LPN in implementing said plansCreating and modifying the Home Health Aide care plan and supervising them via patient feedback and onsite interactionsEvaluating and revising plan of care as patient status and needs changeUsing health assessment data to determine nursing diagnosisDeveloping a plan of care, which establishes goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative action.Administering medications and treatments as prescribed by the physicianIdentifying discharge planning needs as part of the care plan development and implementing prior to discharge of the patientActing as the Case Manager when assigned by the Clinical Supervisor and assuming responsibility to coordinate patient care for assigned caseloadPreparing clinical notes as needed and communicating with the physician regarding patient’s needsBenefits You Will EnjoyIn addition to competitive pay, full-time team member benefits include:Paid Time OffInsurance benefits (medical, dental, vision, life, and other voluntary group programs)401 (K) retirement savings plan with employer matchTuition reimbursement for qualified career pathsCareer advancement opportunitiesScheduleFull TimeEducation, Licensure & Certification RequirementsGraduate of an accredited school of nursing.Active RN licensureOne (1) to two (2) years of recent acute care experience in an institutional settingBachelor’s degree with one (1) year of home health care experience preferredMust be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order #J-18808-Ljbffr
