RN Clinical Documentation Integrity - Putnam, CT

Ensemble is looking for a CDI Specialist based in Putnam, CT at DKH - Day Kimball Healthcare. The role is full‑time, on‑site with a base salary of $80,000 per year. Job Responsibilities Complete initial patient medical record review within 24–48hrs of admission and perform subsequent reviews every 24–48hrs, documenting findings in CDE software. Assign principal diagnosis, CCS/CC/MCC, and evaluate Severity of Illness (SOI) and Risk of Mortality (ROM) for all in‑house patients. Assign working ICD‑10‑CM, PCS codes and DRG using the encoder in CDE software. Clarify missing or unclear documentation with physicians; request and obtain additional documentation as needed. Educate physicians, nurse practitioners, allied health professionals, nurses, and care‑coordination staff on documentation improvement, guidelines, and the importance of accurate records. Collaborate with coding professionals to ensure accuracy of diagnostic and procedural data and completeness of supporting documentation for working and final DRG assignment. Review DRG denial letters and write denial appeal letters. Collaborate with care‑coordination, nursing and ancillary staff to resolve physician clarifications prior to discharge. Maintain and uphold all clinical documentation regulatory guidelines. Submit timely, well‑prepared appeals for reconsideration by third‑party administrators (payors) with supporting documentation. Work collaboratively with coworkers and management to resolve root‑cause issues impacting pay‑or contracts, hospital operations, and reimbursement. Qualifications and Experience Minimum of five years of acute‑care nursing experience in medical/surgical, intensive care, or emergency department settings. Excellent interpersonal, verbal and written communication skills with a strong ability to build physician relationships. Strong organizational skills and ability to present information clearly and concisely. Proficient computer and keyboarding skills; high priority‑setting ability. Inquisitive mindset and openness to innovation, including use of AI to improve processes. Education and Licensure Minimum education: registered‑nurse (RN) licensure (active). CRCR certification required within 9 months of hire. Compensation and Benefits Base salary: $80,000 per year.
