Summary:

This position is responsible for providing care to assigned patients that meets their physical, psychological, social, cultural, spiritual, and developmental needs. Functions as a contributing member of the health care team.

Responsibilities:

• Assists the team leader in assessing the status of patients with simple health problems on admissions and on a continuing basis reporting changes in health status to the RN and / or Physician.
\ N
• Contributes informationto the plan of care.
\ N
• Implements the plan of care, under the direction of the RN and / or Physician and within the scope of practice, to meet the basic needs of assigned patients and families.
\ N
• Documents data for assessments, interventions and effectiveness of care provided in the patients medical record. Findings are reported to the RN and / or Physician.
\ N
• Participates in patienteducation activities as directed by the RN and / or Physician.
\ N
• Assists in the preparation of the patient and family for discharge under the direction of the RN and / or Physician.
\ N
• Administers medicationsto assigned patients in accordance with clinical specific standards.
\ N
• Responds to emergencies.
\ N
• Identifies safety hazards and reports them.
\ N
• Demonstrates competenceto perform assigned patient care responsibilities in a manner that meets the age specific and developmental needs of patients served by the department.
\ N
• Appropriately adapts assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychosocial, cultural, spiritual, age specific and other developmental needs of each patient served.
\ N
• Demonstrates adherence to the CORE values of Santa Rosa Health Care.
\ N
• Perform other duties asassigned.

Requirements:

Education

• High School Diploma or GED Required
\ N
• Graduate from an accredited school of vocational nursing
\ N

\n

Experience

\ N

\n
• Experience in clinical setting preferred
\ N

\n

Licenses, Registrations, or Certifications

\ N

\n
• Licensed as a LVNin Texas
\ N
• BLS Certification required
\ N

\n
