Starting Salary for this position is $48,000 annually, with significant potential for higher earnings based on performance and growth opportunities. This is a full-time Hybrid position. You will be expected to commute to the office at least 2-3 times a week.ResponsibilitiesThis position is a hybrid position. Aspire365 offices are located in Sandy, UtahAccomplishes clients’ care by assessing treatment needs; monitoring, and evaluating treatment plans and progress; monitoring staff performance.Determines clients’ requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plansScheduling clients with Addiction Psychiatrist if neededMonthly scheduled appointments with clients according to appointment schedule.Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources.Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services, obtaining additional resources; intervening in crises; providing personal support.Maintains clients’ records by reviewing case notes, logging events and progress.Communicates clients’ progress by conducting monthly coordination meetings and evaluations with client’s clinician team.Prepares clients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; providing resources.Case Manager Skills and Qualifications:Documentation skillsAnalyzing informationDecision makingResearch skillsVerbal communicationWritten communicationPeople skillsResolving conflictPeople managementIntegrityEducation, Experience, and Licensing Requirements:Bachelor’s degree in social work, Psychology, or related field; or Associate’s degree with at least 2 years Case Management experience; or Equivalent combination of education and experience.Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferredMust be over 21 years of ageJob Type:Full timeWork Location: Hybrid- location in SandySalary: $48,000 per yearE04JI8009u23409azlp #J-18808-Ljbffr
