Care Coordinator

Comprehensive Home Health Solutions (CHHS) – Reno, Nevada

Comprehensive Home Health Solutions (CHHS) is seeking an experienced, compassionate, and highly organized Client Care Coordinator to join our Home Care team.

This is more than a traditional desk-based role. The Client Care Coordinator helps ensure clients receive consistent, high-quality care while supporting, coaching, and developing caregivers. The ideal candidate enjoys working with older adults and families, manages multiple priorities well, and is willing to step in when needed to support client care.

Key Responsibilities

• Develop, maintain, and update individualized client care plans.
• Build strong relationships with clients and families and address questions, concerns, and changing care needs.
• Coordinate with caregivers, Scheduling, Intake, and the Lead Client Care Coordinator to ensure continuity of care.
• Conduct required in-home caregiver supervisory visits and regular client/caregiver check-ins.
• Monitor caregiver performance, professionalism, safety, and adherence to care plans.
• Participate in caregiver onboarding, orientation, training, coaching, mentoring, and competency validation.
• Identify training needs and provide or coordinate additional education.
• Respond promptly to client and caregiver concerns and escalate significant issues appropriately.
• Participate in daily morning huddles when not engaged in a client visit or other time-sensitive responsibility.
• Assist with admissions, staffing transitions, schedule changes, and other operational needs.
• Maintain accurate and timely care coordination, supervisory, and training documentation.
  
  

Schedule

Full-time, generally Sunday through Thursday , with flexibility based on client needs, staffing situations, supervisory visits, admissions, and other operational requirements.

Urgent Shift Coverage

Although this is primarily a Client Care Coordinator position , home care may involve unexpected staffing challenges. On an as-needed basis, the Coordinator may assist with direct caregiving coverage for last-minute call-outs, unexpected vacancies, or other urgent unfilled shifts to help maintain continuity of care.

This role does not have a predetermined percentage of time dedicated to caregiving. The Coordinator is expected to be flexible and understand that when an urgent situation arises, client care comes first. Any direct care provided must be within the employee’s qualifications, scope of practice, and applicable CHHS policies.

Qualifications

• Experience in home care, caregiving, healthcare coordination, or senior care.
• Strong communication and interpersonal skills.
• Highly organized and able to manage multiple priorities.
• Comfortable coaching, training, and mentoring caregivers.
• Able to address concerns, solve problems, and follow through.
• Compassionate, professional, and able to maintain caregiver accountability.
• Flexible and willing to assist with urgent client-care or staffing needs.
• Able to work independently and as part of a team.
• Valid driver’s license and reliable transportation required.
• Able to travel to client homes throughout the service area.

Preferred Qualifications:

• CNA, HHA, Medical Assistant, or other healthcare-related credential.
• Experience with older adults, dementia, chronic illness, complex care, caregiver supervision, or home care operations.

Rate : $23 - $25 - starting rate
