Outpatient Registered Nurse - RN - Dialysis

Purpose and Scope Functions as a Staff Registered Nurse on the hemodialysis health care team to ensure the provision of quality patient care daily in accordance with FMS policies, procedures, and training. Supports the FMCNA mission, vision, values, and customer service philosophy, and participates in quality enhancement and continuous improvement activities to achieve the FMCNA Quality Enhancement Goals. Principal Duties and Responsibilities Directs Patient Care Technicians in the safe and effective delivery of chronic hemodialysis therapy, delegating tasks to LVN/LPNs, PCTs, and Dialysis Assistants. Assesses patients’ daily care needs, develops appropriate assignments, and monitors staff for compliance with facility policies. Participates in new staff orientation, required staff meetings, and ongoing staff training. Provides patient and family education on End Stage Renal Disease, dialysis therapy, vascular access, diet/fluid, medication compliance, laboratory values, and adequacy monitoring (OLC, AMP, UKM). Ensures transplant, modality awareness, and catheter reduction. Implements and documents treatment prescriptions, adjusting therapy as directed by the prescribing physician. Communicates patient issues to the Team Leader or physician and initiates initial and annual nursing assessments. Participates in pre‑evaluation, initiation, monitoring, and termination of hemodialysis treatment, including access homeostasis and post‑evaluation according to FMC procedures. Supervises and monitors hemodialysis access care, manages interdialytic transfusions, and ensures accurate documentation on hemodyalysis treatment sheets. Transcribes physician lab orders into the Medical Information System, prepares lab requisitions, handles specimen packaging and shipping, and reports abnormal lab results. Maintains shift operations safely and efficiently, acts as a resource for staff, and collaborates with physicians and team members to coordinate care. Provides medication administration and ensures controlled substance documentation. Maintains clean and sanitary facility environment, operates dialysis equipment safely, addresses blood spills per FMCNA Bloodborne Pathogen Control Policies, and verifies accurate patient data entry. Manages opening and closing procedures, completes Continuous Quality Improvement activities, and produces monthly nursing progress notes. Completes patient care plans for new or unstable patients, maintains complete medical records, and follows established transplant protocols. Performs additional duties as assigned. #J-18808-Ljbffr
