Discharge Follow-Up Registered Nurse

To provide telephone follow up for the discharged patient, i.e. review of discharge instructions, confirmation of required appointments, answering any questions that the patient/family may have since leaving the hospital, and/or referral to the appropriate person/service, as needed. Permission to receive a courtesy call shall be obtained prior to the patient leaving the hospital. The nurse shall demonstrate accountability for patient satisfaction and service recovery. Estimated pay range for this position is $33.23 - $44.20 / hour depending on experience.

Qualifications

Degrees: Associates. Licenses & Certifications: Basic Life Support. Registered Nurse. Additional Qualifications: Associates in Science required, 3 years acute care experience. Minimum Required Experience: 3 Years
