Hospice RN Case Manager — Compassionate Care Leader
Altus Hospice
Altus Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our dedicated nursing team throughout the Southeast Houston area. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. This role reports to the Clinical Director with a Monday–Friday 8 am–4:30 pm schedule and on-call included.
