MDS Coordinator

Description Hazel Findlay is seeking an MDS Coordinator to coordinate and oversee functions of the MDS and resident care assessment process. We're looking for a compassionate, creative, and resourceful professional to drive our mission. Full-time position available.
Some responsibilities include:

- Interview residents and others for information related to the MDS.
- Review medical records to obtain information related to the MDS.
- Review quality indicator reports for resident concerns.
- Schedule MDS assessments according to state and federal guidelines.
- Schedule and participate in Care Conferences.
- Current LPN or RN license in good-standing with the State of Michigan.
- Current CPR certification.
- RAC certification or ability to obtain within 6 months
- Excellent customer service and interpersonal skills.
- Some experience in the care and management of geriatric residents.
