Job Title: Licensed Clinical Social Worker Department: Integrated Care Location: RRH - 510 Kreag Road Ste 2 Hours Per Week: 40 hours per week Schedule: Day 8:30-4:30pm Summary The Licensed Clinical Social Worker provides Mental Health/Chemical Dependency diagnostic assessment, treatment planning, and evidence-based interventions for people who have mental illness and addiction. Treatment is provided in the context of a multidisciplinary clinical team, supervised by a physician. Leadership and/or supervisory duties as delegated by manager. Provide superior customer service by modeling the Rochester Regional Core Values and professional conduct guided by the NASW Social Work Code of Ethics. Responsibilities Provides initial and ongoing assessment, evidence-based clinical interventions to a designated caseload. Develops treatment plans that are individualized and culturally sensitive. Provides individual, group, and family psychotherapy. Participates functionally as a member of a multidisciplinary team in coordination and delivery of patient care. Communicates in a timely and complete manner with internal and external care providers to complete ongoing assessment and to coordinate care. Participates in and incorporates professional supervision and ongoing education toward quality care. Utilizes efficient computer skills for electronic documentation. Completes all required documentation in a timely and comprehensive manner. Maintains a caseload, meeting productivity requirements as stated by the Department of Behavioral Health, and seeks to keep the incidence of failed appointments to a minimum. Must maintain current licensure as a Licensed Clinical Social Worker (LCSW or LCSW‑R) in NYS. Undertakes planning, problem solving and quality improvement activities regarding program functioning. Pursues personal and professional growth using internal and external sources available for supervision/consultation and continuing education. Provides clinical supervision/consultation/instruction to therapist interns and/or less experienced staff (LMSW or other). Assists Program Coordinator/Manager/Supervisors in assuring smooth, effective delivery of care in response to community requests for services. Credentialing to provide professional billing for clients whose insurer allows for that provision. Required Qualifications Required degree: Master’s in Social Work. Required certification/licensure: Licensed Clinical Social Worker in NYS (LCSW or LCSW‑R). ACT Program and HBCI: Valid NYS driver’s license and participation in NYS LENS program. Required years of experience: Minimum of 3 years’ experience providing clinical social work services. Preferred Qualifications Demonstrates strong communication (both written and verbal), interpersonal, organizational, and time management skills. Experience in CompassionNet: Pediatric and/or palliative care. Education: MS (Required) Licenses / Certifications: LCSW - Licensed Clinical Social Worker, New York State Education Department (NYSED) Physical Requirements: Sedentary work; may involve walking or standing for brief periods; may exert up to 10 pounds of force occasionally. Pay Range: $71,000.00 – $86,000.00 City: Pittsford Postal Code: 14534 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and is eligible for additional pay components.
