Minimum one year of experience. Current state license as a Registered Physical Therapist/PT or be proactively in the process of applying for licensure. Current CPR certification. Other requirements will be determined by the client facility. Duties include providing direct patient care, assessing medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation; establishing and administering a treatment program with specific goals; setting realistic and achievable goals; documenting and recording patient condition; and educating patients and families on appropriate physical therapy methods. Develops a plan of care for each physical therapy patient. Provides skilled physical therapy services/interventions in accordance with physician orders. Assesses patient needs, plans for, evaluates, and modifies care to meet goals of physical therapy interventions. Collaborates with all disciplines to plan and evaluate team goals for each patient.
