Remote RN Care Manager - Telephonic Case Management

Humana Inc. is seeking a remote Telephonic Care Manager to deliver comprehensive case management across the care continuum. You will coordinate with care teams, coordinate with facilities and providers, and serve as the main contact for beneficiaries within the CM program.The role requires an active RN license, 2 years of case management experience, and the ability to obtain CCM within 1 year. Normal hours are M–F, 8:00 a.m.–5:00 p.m. ET or 9:00 a.m.–6:00 p.m.J-18808-Ljbffr
