A highly respected community health organization in Oceanside, California is seeking a Primary Care Physician to join its outpatient care team. Job Responsibilities

✅ Provide comprehensive outpatient primary care to patients across the community
✅ Manage chronic conditions such as diabetes, hypertension, asthma, COPD, obesity, and other common primary care needs
✅ Conduct wellness visits, preventive screenings, follow-ups, medication management, and patient education
✅ Collaborate with APPs, nurses, behavioral health, care coordinators, and other clinical support staff
✅ Support quality initiatives focused on access, prevention, patient engagement, and improved health outcomes
✅ Maintain accurate and timely documentation in the EMR
✅ Provide culturally aware, patient-centered care to a diverse population

Qualifications

✅ Board-certified or board-eligible Family Medicine or Internal Medicine Physician
✅ Active California medical license, or ability to obtain
✅ Outpatient primary care, family medicine, internal medicine, community health, or FQHC experience preferred
✅ Comfortable caring for diverse patient populations
✅ Strong communication skills and a collaborative, team-based approach
✅ Commitment to quality care, access, prevention, and community health
