Home Health Aide- Part Time

Overview Part‑time Home Health Aide position in Lexington region. Provides care and comfort measures to patients in their homes as directed by care plan, interdisciplinary plan of care, and the provider's medical plan of treatment. Responsibilities Review care plan and record visit notes. Document care and significant findings; report concerns or changes to leadership. Assist patients with promoting functional ability and maintaining a comfortable home environment. Provide basic personal care and hygiene; bedside care in a variety of settings. Assist with daily living, personal hygiene (including baths, showers, bed, shampoos, nail care), ambulation, prescribed exercise, and toileting (including use of bedpans and bedside commodes). Apply proper skin care to incontinent patients. Assist with oral or topical medications that the patient can normally self‑administer. Measure fluid intake and output as indicated in the care plan; record visit notes. Report any changes in physical condition, behavior, or appearance to the care team. Qualifications High school diploma or GED. At least 24 months of prior experience as an aide for compensation or completion of a program defined by CMS Home Health Conditions of Participation. Certified as a nurse's aide and registered by the VA Department of Health Professions. Completion of a Geriatric Nursing Assistant, Certified Nursing Assistant, or Home Health Aide course meeting state, federal, and CMS requirements. Successfully completes the HHA Competence Program as established by the home health or hospice agency. Valid driver's license. Compensation Pay range: $15.00 – $21.07 per hour.
