Position Overview

We are seeking a dedicated and experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Cullman, Blount, Winston, and Walker Counties. Key Responsibilities

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing services in accordance with physician orders and agency guidelines.
• Deliver and manage wound care treatments and IV therapy services.
• Monitor patient progress and communicate changes in condition to physicians and care team members.
• Educate patients and caregivers on disease management, medications, treatments, and self-care techniques.
• Coordinate care with physicians, therapists, and other healthcare professionals to ensure optimal patient outcomes.
• Maintain accurate, timely, and compliant clinical documentation.
• Support patients in achieving their health goals while promoting independence and safety in the home environment.

Required Qualifications

• Current Alabama Registered Nurse (RN) License in good standing.
• Experience with wound care and IV therapy/care .
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and patient education abilities.
• Valid driver's license and reliable transportation.
