Growing together. You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The RN is a flexible team member who works alongside nurse practitioners (NPs) and physician assistants (PAs). Primary Responsibilities: Responsible for collaborating with all members of the interdisciplinary care team around changes in condition and/or care coordination Triage member needs to the appropriate care team member and level of care Assist with transitions in care: outpatient and inpatient Assist in assuring complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessments What Makes an Optum Career different? Required Qualifications: Associate degree or higher in Nursing (RN) Active unrestricted Registered Nurse license in your state of residence or ability to obtain within 60 days of hire Certified in Basic Life Support or ability to obtain by start date Driver's license and access to reliable transportation that will enable you to travel up to 100% to visit clients and/or patient sites within a designated area 1+ years clinical experience as a Registered Nurse Preferred Qualification: BSN degree Long term care (LTC) or skilled nursing facility (SNF) experience Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
