The provider will collaborate closely with nephrologists, dialysis teams, hospital staff, and community providers to deliver coordinated, patient-centered care.Key ResponsibilitiesOutpatient Nephrology ClinicEvaluate, diagnose, and manage patients with chronic kidney disease and related disordersDevelop individualized treatment plans focused on preserving kidney function and improving outcomesMonitor laboratory studies, diagnostic testing, and medication therapiesEducate patients and families regarding disease management and treatment optionsCoordinate referrals, follow-up care, and specialty consultations as neededDialysis CareManage hemodialysis and peritoneal dialysis patients in accordance with established clinical guidelinesAssess and treat complications related to ESRD, including anemia, hypertension, mineral bone disease, electrolyte disorders, and vascular access concernsConduct routine dialysis rounding and monitor patient progressCollaborate with interdisciplinary care teams to optimize treatment plansSupport patients and families through treatment decisions and ongoing educationHospital ResponsibilitiesPerform patient evaluations, histories, and physical examinationsParticipate in inpatient nephrology management and care coordinationAssist with admissions, discharge planning, and transitions of careCollaborate with physicians, nurses, case managers, and other healthcare professionals to ensure continuity of careQualificationsRequiredMaster's or Doctoral degree from an accredited Nurse Practitioner programCurrent Massachusetts Nurse Practitioner license or eligibility for licensureCurrent DEA registration or ability to obtainStrong clinical assessment, diagnostic, and patient management skillsExcellent communication and collaboration abilitiesPreferredExperience in nephrology, dialysis, internal medicine, hospital medicine, primary care, or chronic disease managementFamiliarity with electronic medical recordsPassion for caring for patients with complex chronic conditionsAbout Panoramic HealthPanoramic Health is the nation's leading integrated kidney care organization, partnering with nephrology practices across the country to improve outcomes and transform the delivery of kidney care.
