About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty License: Must have license in hand Certifications: CPR, BLS Must-Have: Must have recent specialty experience Description: Provides coordinates and directs the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits required. Periodic office meetings. Must have reliable transportation, current driver's license, and agency-required liability insurance. Average number of patients per day: 5-8. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points. Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. Travel required within a mileage radius of up to 75 miles. Mileage reimbursed at IRS standard rate. Traveler is compensated from one patient home to another. Commute to and from the first and last patient are not billable. Must be proficient with HomeCare HomeBase and have used it within the last 6 months. RNs, PTs, and OTs are required to be proficient in OASIS. Contract hours are not guaranteed. Clinician must hit weekly productivity points of 30, which equates to 40 hours per week. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care will result in a cancellation. Must be open to travel and on-call rotations. Local applicants accepted. All applicants must confirm understanding of the expectations related to patient assignments and documentation.
