Compassionate Hospice RN Case Manager

Ascend Hospice & Palliative Care MA is hiring a Hospice Registered Nurse Case Manager to support On Call/Case Management and patient visits. The RN will assess patient and family needs, re-evaluate nursing care, and help develop and revise care plans. Requires a State Certified RN license, at least 2 years of nursing experience (hospice preferred), ability to travel, strong communication skills, and a commitment to compassionate end-of-life care.
