The Wisconsin Veterans Home at Chippewa Falls is looking for a compassionate and organized Social Worker to join our interdisciplinary team and support the Veterans and spouses who call our community home.

This role provides the opportunity to develop meaningful relationships with members and families while advocating for their needs and helping them navigate the transitions, decisions, and challenges that can come with long-term care.

Schedule & Pay

Full-Time | Monday–Friday | 8:00am - 4:30pm
Starting at $25.00 hour

What Does Social Work Look Like Here?

As a Social Worker, you'll be a resource and advocate for members and their families from admission through their time in our community.

You'll work closely with nursing, therapy, activities, culinary, providers, and other members of the interdisciplinary team to support each member's psychosocial needs, preferences, rights, and quality of life.

Your role may include helping a new member adjust to their surroundings, supporting a family through a difficult change, participating in care planning, coordinating services, addressing member concerns, or helping plan for a safe discharge.

What You'll Do

• Assess and support members' psychosocial needs

• Develop relationships with members and families and serve as a resource throughout their stay

• Participate in interdisciplinary care planning

• Advocate for member preferences, rights, and quality of life

• Assist members and families with adjustment to long-term care

• Provide support related to changes in condition, grief, loss, and other life transitions

• Coordinate services and community resources as appropriate

• Assist with discharge planning and transitions of care

• Address member and family concerns and help facilitate communication

• Maintain timely and accurate documentation

• Support compliance with applicable long-term care regulations and resident rights requirements

What We're Looking For

• Active Wisconsin Social Worker license

• Strong communication and interpersonal skills

• Ability to navigate sensitive situations professionally and compassionately

• Strong organization and documentation skills

• Ability to collaborate effectively as part of an interdisciplinary team

• Long-term care or healthcare experience preferred
