Primary Care Physician (New Orleans) Primary Care Physician role at ChenMed in New Orleans involves independently providing comprehensive care for older adults, managing acute and chronic illnesses, and coordinating care with hospitals, specialty teams, and community resources. ESSENTIAL JOB DUTIES/RESPONSIBILITIES The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient. The PCP takes full accountability for patient care and outcomes and appropriately seeks consultation from specialists when needed, while remaining involved and responsible for the detailed care of the patient. It is expected that the PCP engages with the hospitalist whenever one of their patients is in the hospital, regardless of hospitalist employment status. The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. The PCP leads their care team consisting of a care promoter (medical assistant), a care facilitator, and a care coordinator for patients able to come to the office. For patients unable to come to the office—at hospitals, SNFs, LTC facilities, or homebound—the PCP engages with the transitional care team and other resources available in the market. The PCP has an active role in the management of their center and helps cover for other providers who may be unavailable for various reasons. Each PCP is also expected to take an active role as needed in recruiting patients for the center and additional providers for the company. PCP supervises, collaborates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company. Performs other duties as assigned and modified at manager’s discretion. EDUCATION AND EXPERIENCE CRITERIA MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is required. Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which the PCP will be working. Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred; board eligibility is required. Once board certified, the PCP will maintain board certification in their specialty by completing necessary MOC, CME, and/or retaking board exams as required. Must have a current DEA number for schedule II–V controlled substances. Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross is required within the first 90 days of employment. #J-18808-Ljbffr
