Minimum Requirements

• Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA.
• Active West Virginia Physical Therapist license required to start the assignment.
• Home health experience required. Experience providing in-home patient care in home health or hospice settings preferred.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Familiarity with home health documentation and point-based productivity expectations. Client expects approximately 30 productivity points per week.
• Ability to travel and provide patient visits across Kingwood, WV and the surrounding two counties. Mileage between visits will be reimbursed.
• Reliable transportation and valid driver’s license required for community-based visits.
• Preferred: Experience with HCHB electronic medical record system.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Monday through Friday, 8:00 AM – 4:00 PM (daily schedule per client: M-F 8:00–4:30 PM; documented shift detail: Days 0800–1600).
• Weekly Hours: 40 hours guaranteed as documented by the client.
• On-call/Weekend Requirement: No weekend or on-call requirement.
• Coverage Area: Kingwood, WV and surrounding two counties. Candidate must be prepared to travel within this coverage area.
• Primary responsibilities: Provide direct patient care in patients’ homes, complete required documentation and meet productivity expectations.
