Role Description This is a full-time, on-site Nurse Practitioner role based in Chicago, IL. The Nurse Practitioner will provide comprehensive wound assessments, develop and implement treatment plans, and perform procedures such as debridement and dressing changes in patient care facilities or residential settings. Responsibilities include monitoring healing progress, adjusting care plans, documenting clinical encounters, and coordinating with physicians, nurses, and facility staff to ensure continuity of care. The role involves educating patients and caregivers on wound prevention and self-care, supporting infection control practices, and contributing to quality improvement initiatives and clinical protocols within the practice. Qualifications Active Nurse Practitioner license in Illinois and national NP certification; current BLS/CPR certification. Clinical expertise in wound care, including assessment, treatment planning, and performance of wound-related procedures. Strong patient-centered communication skills, with the ability to educate patients, families, and care teams clearly and empathetically. Proficiency in clinical documentation, electronic health records, and adherence to regulatory and compliance standards. Ability to work independently in mobile and on-site settings, manage time effectively, and adapt to changing clinical environments. Experience in long-term care, home health, or outpatient specialty care is preferred. Master’s degree in Nursing or higher from an accredited program; wound care certification (e.g., WOCN, CWS) is a plus. #J-18808-Ljbffr
