Travel Registered Nurse - Case Manager
Location: Englewood, CO
Assignment Duration: This is a 13-week contract position.
Schedule: Day Shift: 8:00 AM - 4:30 PM, Friday to Monday
Job Description: Join a dedicated team of healthcare professionals as a Travel Registered Nurse specializing in Case Management. You will play a crucial role in managing patient care and discharge planning while working in a fast-paced environment.
Responsibilities:
- Conduct assessments and develop discharge plans.
- Manage patient care from admission to discharge.
- Collaborate with the healthcare team to ensure optimal patient outcomes.
- Maintain accurate and timely patient records in EMR systems.
- Educate patients and families on post-discharge care.
Required Experience / Certifications / Licensure:
- Minimum 1 year of current Case Management RN experience.
- Experience in discharge planning, emergency department, or palliative care is preferred.
- Current RN License in the state of practice.
- Strong communication and adaptability skills.
