Look no further than Saint Luke's!***## ### **Job Details:*** ### Full-time; Monday-Friday 7:30am - 4:00pm; In-Person* ### Provides social work services to patients in an acute care setting for patients with acute, chronic or terminal illnesses and their families* ### Services to patients and their families include: identification of psychosocial needs, psychosocial assessment, discharge planning, crisis intervention, information and referrals, protective services, and patient/family education* ### Functions as a member of a multidisciplinary team for patient care.
