Responsibilities Conduct annual wellness visits and manage high-risk patients, primarily at the patient's residence, supplemented by telehealth. Thoroughly document medical histories, symptoms, and medication information. Administer various tests and screenings. Collaborate with our Primary Care team to ensure holistic patient care. Ensure timely completion of patient records, with access to electronic medical records and portals. Provide patient education on preventive care and prescribed treatments. Maintain strict confidentiality in line with HIPAA regulations. What You Can Expect This is a full-time Nurse Practitioner opportunity in clinic. The position requires flexibility across regions to provide coverage as needed to accommodate scheduling changes. Experience and Qualifications Nurse Practitioner certification from ANCC or AANP. 2+ years clinical experience as an NP in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care. Home care experience is a plus. Working knowledge of chronic care management and medication options. Current DEA licensure / Prescriptive Authority post-hire. Active CPR/BLS Certification.
