- Title: Registered Nurse (RN) – Primary Care / Outpatient - Location: Dual Location - San Jose, CA 95128; Moorpark Avenue, Suite 300 - San Jose, CA 95128; N. Jackson Avenue #1 & #4 - San Jose, CA 95116 - Pay: $55.21 – $64.50/hour (Based on experience) + Benefits - Full-Time | Permanent | Direct Hire Job Summary: We are seeking an experienced Registered Nurse (RN) with Primary Care or Outpatient clinic experience to join a collaborative healthcare team in San Jose, CA. This is an excellent opportunity for an RN who enjoys providing patient-centered care in a fast-paced ambulatory setting while working closely with physicians and interdisciplinary care teams. Responsibilities: Deliver direct patient care in a primary care/outpatient clinic. Triage patient calls and walk-in visits. Review laboratory and diagnostic results and coordinate follow-up care. Develop and implement individualized care plans. Coordinate referrals to specialty providers. Educate patients on chronic disease management, wellness, and preventive care. Collaborate with physicians, behavioral health providers, and care management teams. Maintain accurate documentation in the Electronic Health Record (EHR). Ensure compliance with HIPAA, clinic policies, and nursing standards. Participate in quality improvement initiatives and patient safety programs. Required Qualifications: Active California Registered Nurse (RN) License. Bachelor of Science in Nursing (BSN). Minimum 2 years of recent Primary Care or Outpatient nursing experience (strongly preferred). Current BLS Certification. Experience with EHR documentation. Strong assessment, communication, and care coordination skills. Preferred Qualifications: ACLS and/or PALS Certification. Bilingual in Spanish, Mandarin, or Vietnamese. Experience with chronic disease management and preventive care.
