On Call Schedule: Evey other week/weekend
License Requirements: SC RN license
Productivity Requirement: 30 units
Mileage reimbursement: 0.48 cents per mile

Duties :

• Conduct comprehensive patient assessments, develop and implement individualized care plans, provide case management, and monitor patient progress in collaboration with physicians and the interdisciplinary care team.
• Coordinate and oversee medication management, ensure accurate clinical documentation, communicate changes in patient condition, and maintain compliance with healthcare policies and standards.
• Supervise and support LPN/LVN and unlicensed staff, provide health and safety training, and ensure timely follow-up on medical appointments, hospitalizations, and physician orders.
• Promote infection prevention, health maintenance, and disease management while ensuring a safe care environment and high-quality patient outcomes.
