POSITION SUMMARY

Under the direction of the Sr. Director of Housing, the Clinical Client Care Coordinator (LMSW/LMHC) is accountable for providing intensive case management and crisis intervention services for clients. The Clinical Client Care Coordinator will work with individuals and families to promote recovery, resilience, wellness, and long-term housing stability through trauma-informed care, clinical assessment, counseling, crisis intervention, case coordination, and individualized service planning. The position will work closely with Iris House staff, housing personnel, government agencies, healthcare providers, behavioral health providers, and community-based organizations to ensure coordinated and client-centered service delivery

ESSENTIAL POSITION FUNCTIONS

Clinical Services and Assessment

·  Conduct comprehensive psychosocial and resident assessments to identify client strengths, needs, risks, and service priorities.

·  Develop and maintain individualized service plans in collaboration with clients.

·  Provide short-term individual counseling, supportive counseling, crisis intervention, and other appropriate clinical services.

·  Provide trauma-informed, culturally responsive, and client-centered services to clients.

·  Identify and respond appropriately to high-risk situations involving mental health concerns, substance use, safety concerns, and other behavioral health needs.

·  Monitor client progress and adjust service plans and interventions based on changing needs.

·  Advocate for clients and assist them in navigating healthcare, behavioral health, housing, public benefits, and other service systems.

Supportive Housing and Resident Services

·  Maintain an active caseload of approximately 8–12 clients.

·  Coordinate client move-ins, tenant orientations, wellness check-ins, and other supportive housing activities.

·  Conduct regular resident assessments and monitor ongoing progress toward individualized goals.

·  Work closely with the Housing Specialist and other housing staff to address tenancy-related concerns and promote housing stability.

·  Provide crisis intervention, conflict resolution, mediation, and problem-solving support when needed.

·  Assist clients with understanding and maintaining compliance with housing subsidy requirements, recertification’s, rent-related concerns, and other housing program obligations.

·  Support clients in developing skills and strategies necessary to maintain stable housing and increase independence.

·  Identify potential tenancy risks and intervene early to prevent housing instability.

Care Coordination and Community Linkage

·  Connect clients with appropriate mental health, medical, substance use, educational, employment, financial assistance, benefits, and other community-based services.

·  Develop and maintain strong working relationships with external service providers and community partners.

·  Coordinate referrals and follow up to ensure clients successfully access recommended services.

·  Collaborate with government agencies, healthcare providers, behavioral health providers, social service organizations, and other community partners.

·  Participate in interdisciplinary meetings and case conferences as appropriate.

·  Ensure continuity of care and appropriate transitions to long-term community-based services.

Groups, Workshops, and Educational Programming

·  Facilitate therapeutic, supportive, educational, and wellness-focused groups.

·  Develop and/or utilize appropriate group curricula based on identified client needs.

·  Facilitate workshops and educational programming designed to promote wellness, recovery, resilience, independent living skills, and housing stability.

·  Provide group-based educational programming at least twice annually, or as otherwise required by the program.

·  Evaluate participant engagement and provide appropriate follow-up and referrals.

Documentation and Compliance

·  Maintain accurate, timely, and complete clinical and case management documentation in accordance with Iris House policies, program requirements, contractual obligations, and applicable regulations.

·  Complete case notes, assessments, service plans, progress notes, reports, referrals, and other required documentation.

·  Maintain confidentiality and protect client information in accordance with applicable laws, regulations, and organizational policies.

·  Participate in program monitoring, quality assurance activities, audits, and required reporting.

OTHER POSITION FUNCTIONS

1.       Supervise interns

2.    Performs other related duties as assigned.

POSITION REQUIREMENTS

Education:                          Master’s degree in Social Work, Mental Health Counseling, Counseling, or a related clinical field from an accredited institution.

LMSW or LMHC licensure is required within 90 days of employment, in accordance with applicable requirements

Experience:                             Experience providing clinical services, counseling, case coordination, or supportive services to individuals and/or families.

Experience conducting psychosocial or standardized assessments.

Experience facilitating groups and developing or implementing group curriculum.

Other Requirements:          Excellent verbal and written communication and organizational skills.  Knowledge of special needs of individuals and providing services to those infected with HIV/AIDS.  Experience with evidenced based practices and in particular motivational interviewing. Bilingual (Spanish/English) preferred.

Internal:                             Frequent contact with Iris House clients and department staff members; occasional contact with visitors.

External:                            Frequent contact with contract provider.  Occasional to frequent contact with community based organizations, medical providers and/or other agencies.
