Internal Medicine Nurse Practitioner

Nurse Practitioner Job Summary:

Conduct daily rounds on patients in a post acute care environment.
Collaborate with a multidisciplinary team to assess and address patient needs.
Develop and implement individualized care plans based on patient assessments.
Monitor and manage patient progress, making necessary adjustments to treatment plans.
Provide education and support to patients and their families regarding their healthcare needs.
Maintain accurate and up-to-date patient records.
