Travel RN Case Manager - $2,655 per week

LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Mission Viejo, California.

Job Description & Requirements

Requirements

• Active California RN License
• 2+ years of recent Acute Care Case Manager experience performing Care Coordination (within the last 3 months)
• Travel Case Management experience required (First-time travelers are not accepted )
• Epic EMR experience ( Hard Stop )
• Experience with:
  • Care Coordination
  • Discharge Planning
  • Patient Assessment
  • Family Meetings
  • Treatment Plan Development
  • DME, SNF, and Ambulance Transport Arrangements
• Ability to work a variable 5-day schedule , including weekends as needed
• Willing to float to the Laguna facility
