RN Assessment Coordinator - FT - Days - Rehab RN Assess Coord - 17287 US:TX:Edinburg | REHABILITATION SERVICES | Full Time Posted a week ago Overview Assessment Coordinator shall be responsible for providing administrative support to the patient admission process and will contribute to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary fashion. Qualifications Graduate of accredited school of nursing required. Current active registered nurse licensure in the State of Texas required. Current BLS Certification required. Valid Texas Driver’s License. Willingness to be cross trained to other areas. Organizational skills are necessary. Good written and verbal communication skills. Ability to read, write, and speak English. Ability to communicate clearly and concisely with all levels of nursing, administration, and physicians. Preferred hospital experience. Three to five years experience in a health care setting strongly preferred. Strong communication and organizational skills. Reasoning ability and good independent judgment. Professional image. Ability to work with frequent interruptions. Responsibilities Promote the facility mission, vision and values and consider them in developing services, standards, and practice. Arrange for pre‑admission screening and verify screening arrangements within 30 minutes of receiving referral. Assist in orienting patient and families to unit. Ensure pre‑admission screening is completed according to policy & procedure. Determine eligibility within one business day of referral and offer alternate placement suggestions if needed. Complete verification of admission pre‑certification and insurance benefits if patient is to be admitted. Coordinate bed availability with Nurse Manager / Program Director. Develop network with case managers for referrals and aid in discharge planning. Communicate with hospital case managers daily regarding potential post acute needs of acute patients via phone, face‑to‑face, or e‑mail. Conduct center tours for patients and families. Attend team business meetings and offer ideas/solutions in problem‑solving situations/activities regarding the program. Function as a resource for and provide team members with in‑service/cross‑training on admissions knowledge and skill areas. Contribute to and cooperate with the implementation of system improvements. Assist other staff and demonstrate flexibility in carrying out shared team functions including special projects. Complete tasks as requested. Utilize resources appropriately and maintain equipment to prolong working life. Attend scheduled unit quality management meetings as assigned. Submit data as requested. Assist with coverage for Case Management/Social Work as needed for inpatient and outpatient Rehab (if applicable). Adhere to safety standards for staff and patients, and encourage compliance by others. Other duties as assigned. Customer Service Provide excellent customer service to all DHR customers.
