Neighborhood Healthcare in Riverside, CA is seeking a PACE Registered Dietitian to design and implement nutrition education programs, evaluate nutritional status, develop care plans, and communicate nutrition information to providers.
This role supports diabetes education, dietary counseling, and group education activities while ensuring adherence to CPSP and PACE guidelines and maintaining accurate documentation in the EMR.
