Description:

The RN implements professional nursing practice centered on the patient as a whole person. The RN creates a caring environment focused on healing and wellness. Nursing practices are based on evidence and current research as well as hospital and department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit through nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also part of the role. The RN provides patient and family-centered education, care coordination, and individualized treatment planning as applicable. The RN performs and documents comprehensive assessments in accordance with the RN's scope of practice and hospital policies/procedures. The RN considers the patient's culture, age, and developmental level. Identifying patient risk factors and presenting complaints is essential. The RN obtains patient health information from family or other necessary persons. Collecting objective/quantitative and subjective/qualitative data is part of the assessment process. The RN verifies initial admission assessments if obtained by other members of the healthcare team. Developing a problem list and prioritizing patient needs based on input from the inter-professional team is required. Creating actions and interventions relevant to the patient's culture and age is necessary. The RN determines desired outcomes in coordination with the patient and family in accordance with the medical plan of care. The RN fully implements patient care therapies and procedures with empowerment and clinical autonomy within the scope of practice. Providing explanations of procedures, treatments, and processes to patients and families is essential. The RN initiates action to alleviate the patient's stress and discomfort. Recognizing, responding to, and communicating changes in patient conditions appropriately is a key responsibility. The RN conducts and documents ongoing assessments and responses to procedures and therapies using the nursing process according to hospital policy and established standards. Delegating tasks appropriately and overseeing team members in the performance of patient care activities is part of the role. The RN monitors the accuracy and completeness of delegated patient care provided by other team members. Identifying and addressing educational needs of patients related to prioritized problems according to their level of comprehension is important. Facilitating the discharge process by executing discharge orders in a timely manner is a responsibility. The RN provides input into patient/nurse assignments considering staffing, unit activity, and patient acuity. Actively participating in unit-based activities in support of organizational values and priorities is expected. Promoting a positive group dynamic and healing environment through words and actions is important. Assuring nursing excellence in accordance with accepted standards, evidence-based practice, current research, and caring philosophy is fundamental. The RN actively participates in department and/or unit quality improvement activities and core measures. Using a holistic perspective regarding patient care is an essential part of the role.
