Case Management Overview

Washburn Center for Children case managers work collaboratively with children, families, and multidisciplinary teams to develop individualized care plans and ensure access to needed services. These services may include mental health, social, educational, health, vocational, and recreational supports.

Case managers advocate for the needs of children and families and coordinate care across multiple providers and systems. Services are provided to children and youth ages 3 through 17 in home, school, and community-based settings.

Case Management is a unique and rewarding field that offers a comprehensive understanding of family mental health systems. This role is well suited for individuals with Social Work experience or Mental Health-qualified professionals.

This position is highly collaborative and provides opportunities to work across all Washburn Center programs, partnering with experienced professionals to ensure the highest quality of care for children and families.

Case managers are actively engaged in the community and in clients' home environments. Independent work is encouraged to support responsive, client-centered care while remaining connected to a supportive team structure.

Role Responsibilities

• Manage a caseload of 15-17 children or youth and their families, meeting all associated service and documentation expectations.
• Provide culturally responsive, individualized Children's Mental Health Case Management services in homes, schools, and community settings, based on each family's unique needs.

Coordinate mental health and related health services to:

• Improve or maintain a child's emotional and behavioral functioning
• Support families in accessing needed resources, including third-party insurance or Medical Assistance
• Reduce the risk of out-of-home placement, maltreatment, or involvement with protective services

Complete timely and accurate documentation, including:

• Individual Family Community Support Plans (IFCSPs)
• Functional Behavioral Assessments (FBAs)
• Progress notes
• Case management authorizations

Day-to-Day Activities

Treatment & Family Support
Case Managers provide education and support to empower families to self-advocate and confidently navigate complex systems.

Assessment
Complete functional assessments to identify the medical, social, educational, and community-based resources needed to support the child's mental health.

Service Planning
Develop an Individual Family Community Support Plan (IFCSP) based on assessment findings and diagnosis.

Care Coordination
Depending on each case, refer to, coordinate, and monitor services to ensure continuity and effectiveness of care.

Compliance Requirement

All clinicians and case managers must comply with Minnesota data privacy laws and HIPAA requirements at all times.

Requirements

• A Bachelor's degree in one of the behavioral sciences or related field is required
• A Bachelor's degree in Social Work is strongly preferred
• If you hold a Bachelor's degree in Social Work, holding a LSW is required
• A minimum of 1 year or 2,000 hours of supervised of experience working with children and families impacted by mental health challenges is strongly preferred
• Experience working independently in community settings is strongly preferred
• A valid driver's license and access to reliable transportation is required
• Demonstrated knowledge of cultural diversity and the role of culture in service delivery is required
• Flexible, resourceful approach to work, with experience or in-depth knowledge of home and community-based services is required
