Field Community Health Worker - Outreach & Resources
AbsoluteCare
AbsoluteCare is seeking a Community Health Worker to connect members with resources addressing social needs and barriers to care. You will meet members where they are—home, community, hospital, or clinic—and enroll them in internal and external services, educating them about AbsoluteCare. You will operate in a matrixed environment reporting to the Community Team Care Manager daily and to Engagement Manager/Director for program oversight, performance, and alignment. #J-18808-Ljbffr AbsoluteCare
