Major Duties

Conducts clinical reviews to interpret and evaluate complex data directly related to diagnosis, condition, and treatment plan to assure appropriate medical necessity, care coordination, and timely utilization of CCN health care services.

Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care within the CCN.

Organizes and coordinates the clinical components for the authorization of care within the CCN in response to individual healthcare needs along the illness and care continuum.

Centers services around the patient, to foster patient self-managed care, and maximize efficient and cost-effective use of health care resources.

Utilizes all avenues of resources available to provide safe and efficient timely coordination of quality health care and services.

Completes clinical medical records review within specified time limits of receipt.

Completes request for service (secondary authorizations) clinical review within specified time limits of receipt and manage requests until completed.

Collaborates with Referral Coordination Team and manages entire episode of care in the community to re-integrate veteran back to VA services.

Educates Veterans and family/caregivers on care coordination process.

Communicates with patient and serves as consistent point of contact throughout episode of care.

Coordinates ancillary services provided by the VA in conjunction with episodes of care in the community (pre-op clearance, durable medical equipment requests, rehab services, etc.).

Conducts comprehensive assessment of the Veteran and the Veteran's family/caregiver needs.

Collaborates with internal VA providers and community providers.

Completes all required clinical notes during life of consult.

Manages, coordinates, and completes Emergency Care Notifications to re-integrate back into the VA.

Other duty and assignment to meet the needs of service to ensure timely veteran care.
