This is a Home Health Pay Per Visit role.

Position Summary:


As a Home Health LVN, work as a vital member of the care team, providing hands-on support to patients while utilizing AI-supported technology to simplify your documentation process. Spend more time delivering care and less time on paperwork.

Key Responsibilities:

• Assist with medication management, wound care, and health monitoring.
• Use AI systems to assist in completing clear and accurate notes during visits.
• Focus on patient interaction and quality of care by reducing administrative burden.

Education and/or Experience

• Graduate of an accredited Licensed Practical/Vocational Nurse program.
• 1 year experience in a hospital, nursing home, home health, or experience working with terminally ill patients.

Certifications, Licenses, and Registrations

• Education: Must meet certification, licensure, or registration requirements.
• Work Experience: Minimum 1 year as an LVN (6 months home care preferred).
• Certifications/Licenses: California Licensed Vocational Nurse required upon hire; Infusion and Venipuncture Certification required; National Provider BLS - American Heart Association within 30 days of hire. Valid driver's license and auto insurance required upon request.

Pay Range: $33.05-$40.82
