Physical TherapistA dedicated Physical Therapist is needed to join an interdisciplinary team focused on providing high-quality, patient-centered care. This role is crucial in coordinating and delivering rehabilitative services to enhance participant independence and mobility, whether at the center or in-home settings.Key Responsibilities:Conduct comprehensive Physical Therapy assessments in both facility and home environments.Develop and implement customized care plans in collaboration with an interdisciplinary team.Provide active and passive therapy interventions, including muscle re-education, gait training, and ADLs (Activities of Daily Living).Utilize treatment methods such as massage, heat, sound, water, light, or electrical therapy.Supervise Therapy Assistants, offering training and guidance for Functional Maintenance Programs (FMP).Educate staff, caregivers, and family members on therapeutic equipment usage and patient mobility strategies.Qualifications:Bachelor's Degree in Physical Therapy with at least 5 years of experience.Master's or Doctorate in Physical Therapy (Preferred).Valid California Physical Therapy License or eligibility to obtain one.Minimum of 2 years of experience in a clinical setting, preferably working with frail or elderly populations.CPR certification preferred.If educated outside the U.S., the Physical Therapy degree must be evaluated for U.S. equivalency.Compensation & Benefits:Base Salary: $100,000 - $120,000 (DOE)Full Benefits Package, including:Medical, Dental, and Vision Insurance401(k) Savings Plan with Matching17 Days of PTO + 12 Paid Holidays + Sick TimeWork-Life Balance Focused EnvironmentRelocation Assistance Possible for Ideal Candidate
