LPN Unit CoordinatorRate: $30-$32/hour. Sign-on bonus: Up to $3,000. Shift: Full-Time. Schedule: Days, 7 am- 3:30 pm.Harris Hill Nursing Facility is looking to add an LPN Unit Manager to its nursing leadership team. In this role, you'll oversee the daily clinical operations of an assigned nursing unit, ensuring high-quality resident care, regulatory compliance, and effective coordination among residents, families, providers, and interdisciplinary team members. Our team is known for its collaborative culture, strong resident relationships, and dedication to helping both residents and employees thrive.Responsibilities:Oversee the clinical care and daily operations of an assigned nursing unitSupervise nursing staff, nursing assistants, and unit support staff while providing leadership and guidanceMonitor resident conditions, review assessments, and ensure care plans are updated and implemented appropriatelyCoordinate communication with physicians, nurse practitioners, physician assistants, residents, and familiesReview laboratory results, provider orders, incident reports, and changes in resident conditionParticipate in care plan meetings, QAPI activities, family meetings, and interdisciplinary team discussionsAssist with admissions, discharges, transfers, and coordination of outside appointmentsEnsure unit supplies, documentation, and resident records are accurate and currentSupport staff education, orientation, infection prevention efforts, and quality improvement initiativesAddress resident, family, and staff concerns while promoting a positive care environmentPerform additional duties as assignedRequirements:High School Diploma or equivalent required; Associate's Degree preferredValid NYS Licensed Practical Nurse (LPN) License requiredMinimum 1 year of LPN experience in a long-term care setting required3-5 years of long-term care experience with supervisory experience preferredStrong knowledge of resident care planning, clinical documentation, and long-term care regulationsLeadership experience with the ability to supervise, coach, and mentor staffExcellent communication, organizational, and problem-solving skillsAbility to coordinate care across multiple disciplines and manage competing prioritiesCommitment to providing compassionate, resident-centered careBenefits:Paid Time Off (PTO) for Full-Time and Part-TimeWeekly or Same-Day Pay OptionsHealth, Dental, and Vision InsuranceLife InsuranceEmployee Referral Bonus ProgramRetirement Saving OptionsQualifications:Education: High School or better.Licenses & Certifications: CPR.
