• Develop, update, and oversee individualized care plans
• Supervise and support CNAs, GNAs, HHAs, and Med Techs
• Monitor changes in patient condition and coordinate follow-up care
• Communicate with families, physicians, and care teams
• Ensure safe, compliant, high-quality care delivery
• Maintain accurate and timely clinical documentation

• CPR/First Aid certification
• Reliable transportation for home visits
• Strong clinical assessment and communication skills
