Compensation $80 initial visit, $65 follow‑up visit, $70 discharge visit. Mileage reimbursement available. About Gurwin Healthcare System Gurwin Healthcare System is a full continuum of care facility located on a 36‑acre campus in Commack, NY. It includes the 460‑bed Gurwin Jewish Nursing & Rehabilitation Center offering short‑term rehabilitation, long‑term skilled nursing, memory care, respiratory and ventilator care, palliative and hospice care, adult day programs, and on‑site dialysis. The campus also hosts Gurwin’s Assisted Living community (Gurwin Jewish: Fay J. Lindner Residences), the independent living community (Fountaingate Gardens), and certified/licensed home‑care agencies. Position Summary We are seeking a Speech Language Pathologist for our Home Care Division with adult experience who will embody the passion and commitment required to deliver excellent care to our patients. Work Schedule Per Diem (as needed). Preference for coverage in Nassau County and/or Eastern Suffolk, but open to other towns as well. Education and Experience Requirements • Current New York State license to practice Speech Pathology. • Minimum of two years experience as a Speech Pathologist. • Home‑health experience is preferred. Physical Work Requirements • Ability to travel to patients’ homes to make visits. • Ability to make clinical and managerial decisions within the scope of professional practice to perform speech therapy procedures competently and independently. Primary Responsibilities Report the patient’s initial status, significant changes, therapy plans and goals, ongoing status, and discharge plans. Provide the agency with timely documentation, including assessments, plans, goals, visit care, summary reports, and billing/visit data. Instruct patients, families, and team members regarding programs that enhance and facilitate care and follow‑up. Evaluate the patient’s social and architectural environment, make recommendations, and implement them as feasible. Comply with all regulatory criteria. Notify the agency of visit schedule and changes. Provide professional leadership, clinical guidance, and expertise to team members. Attend regular continuing education programs. Perform related duties, as necessary. Adhere to all aspects of customer service, including teamwork, empathy, courteous communication, and respect of others.
