Registered Nurse Case Manager
As a Registered Nurse Case Manager, you will provide care for patients across acute care departments, managing 16-20 patients at a time. Your responsibilities will include discharge planning and collaborating with families on care plans. This role offers a structured schedule with no on-call requirements and support from Case Management Assistants and Social Workers.
