The position is in an outpatient care setting and centers on collaborative, interdisciplinary care to restore functional abilities.

Responsibilities
- Assess patients’ PT needs in an outpatient setting and develop individualized treatment plans
- Implement therapeutic interventions to improve mobility, strength, balance, gait, and functional activities
- Monitor progress, adjust treatment plans as needed, and document outcomes
- Collaborate with an interdisciplinary team to coordinate care and participate in group patient sessions
- Educate patients and caregivers on home exercise programs, safety, and self-management
- Document accurately in the electronic health record and support compliant, outcomes-focused care plans

Requirements
- Active Physical Therapist license in California (or eligible for licensure)
- Doctor of Physical Therapy (DPT) or equivalent degree
- Experience or comfort working in an outpatient physical therapy setting
- Ability to work 8-hour days, 5 days per week (40 hours) and participate in a travel assignment
- Strong communication and documentation skills
- Demonstrated ability to collaborate with an interdisciplinary team
