Job TitleProvides direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care in an ambulatory setting.Job DetailsFull-Time40 hours per weekTuesday-Friday, 7:30 a.m- 5:30 p.m.No weekends or holidaysBenefits eligible from day one!Job ResponsibilitiesDevelops and implements individual plans of care in collaboration with other members of the healthcare team in accordance with the established guidelines and standards of nursing care. Promotes patient and family centered care in a healing environment.Executes nursing interventions by collaborating with the interprofessional team incorporating appropriate standards of care and practice and patient outcomesAssesses and systemically collects any data related to patient health status and works in collaboration with the care team to develop the plan of careEvaluates patient's response to plan of care and works in collaboration with the interprofessional team to make any needed adjustmentsAssist with the coordination of care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools).Assist with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellnessMay float to other clinics on as needed for operations needPerforms Other Responsibilities as Assigned.Minimum Qualifications• Degree: Nursing Diploma/Associate's Degree in Nursing• Experience: No Experience• Supervisor Experience: No Experience• License: RNPreferred Qualifications• Degree: Bachelor's Degree in NursingWork ShiftDay Shift (United States of America)Legal StatementThe above information on this description has been designed to indicate the general nature and level of work performed by employees in this position.
