Requirements **Required for presentation: Must have actual state license, board certification in hand, and active DEA for the state of application and CSR, if required by state, in hand.** EMR-EPIC Education/Training: Must have completed residency in EM, IM or FM Clinical Experience: N/A License: Active NY medical License and DEA w/ NY address & schedules 2,2N,3,3N,4 & 5 Board Certification: Must be boarded w/in 5 years of completing residency Merit Badge: FM/IM=ACLS and PALS (AHA) and ATLS. BE in EM must have ACLS, PALS and ATLS until boarded Immunizations: Current Flu (every year) | Current PPD or Chest Xray (for initial credentialing) | Current Physical (for initial credentialing) | Current N95 Fit Test (every year) | Titers for MMR-Hep B-Varicella Case Log: past 2 years CME/CEU: Past 2 years - no number of credits needed Specific Facility Procedures: Moderate Sedation Malpractice Coverage: $2.3M/$6.9M Additional Details: N/A
