The ideal candidate will provide support primarily to patients receiving hospice care, which includes individuals experiencing terminal illnesses and their families. This role requires sensitivity, empathy, and an understanding of the complex emotions related to end-of-life care, ensuring that patients and families feel heard, supported, and empowered during their journey.

• A Master's degree in Social Work from an accredited program.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
