Job Overview

The RN/LPN in a senior living/SNF/LTC setting provides comprehensive, compassionate, and individualized care to residents, ensuring their physical, emotional, and psychosocial needs are met. This role requires clinical expertise, strong communication skills, and the ability to work collaboratively with a multidisciplinary healthcare team


Work Setting: Skilled Nursing Facility (SNF) or Long-Term Care (LTC) senior living community providing extended care for residents with chronic illnesses, disabilities, or post-acute recovery needs.

Key Responsibilities

• Assessment & Planning: Evaluate resident health status, develop, coordinate, and implement individualized care plans in collaboration with physicians, therapists, and case managers
• Clinical Care: Administer medications, perform vital sign checks, monitor medical conditions, and provide therapeutic interventions such as range-of-motion exercises, wound care, and mobility assistance
• Daily Living Support: Assist residents with activities of daily living (ADLs) including bathing, dressing, eating, and toileting
• Patient Monitoring: Continuously assess and document resident health status, report changes promptly, and respond to emergencies
• Education & Support: Provide health education to residents and families, offer emotional support, and promote independence and dignity
• Team Collaboration: Work closely with physicians, nurses, therapists, social workers, and other staff to ensure safe, coordinated care
• Documentation: Maintain accurate, timely, and complete medical records in compliance with facility and regulatory standards
• Environment & Safety: Maintain a clean, safe, and organized work environment, adhering to infection control and safety protocols

Required Qualifications

• License: Active LPN/RN license
• Experience: Minimum experience in long-term care, skilled nursing, or senior living settings; experience with geriatric care preferred
• Skills: Strong clinical judgment, patience, empathy, communication, and ability to work in a team-oriented environment
