The candidate should possess skills in discharge planning and the documentation of care plans. Strong communication skills will support collaborative care efforts. Understanding of Medicaid and social support programs is preferred. Experience working with diverse patient populations is essential. Candidates should be prepared to float to other facilities as needed. The LCSW will develop individualized plans of care to support patients and their caregivers. All responsibilities must be performed while complying with privacy and ethical standards.
